CHRONIC DISEASE MANAGEMENT PLAN:
Children may be eligible to receive a rebate for up to five sessions under the Medicare Chronic Disease Management Plan (also known formerly as an Enhanced Primary Care Plan).
Eligibility Criteria:
To be eligible for this plan, you will require a Team Care Arrangement Document provided by your GP. This is to be emailed or provided to our admin team prior to the first appointment using this plan.
Your GP will also validate that your child has:
- A chronic condition; and
- Complex care needs being managed by a medical practitioner (including a general practitioner, but not a specialist or consultant physician) under a shared care plan or under both a GP Management Plan and Team Care Arrangements
