CHRONIC DISEASE MANAGEMENT PLAN:
Children may be eligible to receive a rebate for up to five sessions under the Medicare Chronic Disease Management Plan (also known formerly as an Enhanced Primary Care Plan) in a calendar year.
Please note that you will be unable to claim the rebate directly from OT Kidz, and will be required to pay the full amount. You will receive an electronic copy of your paid invoice which contains all the details required for submission to Medicare. You can claim the rebate back either in person at a Medicare center, online, or via the Medicare smartphone app.
Eligibility Criteria:
To be eligible for this plan, you will require a Team Care Arrangement Document provided by your GP. This is to be emailed or given in person to your Occupational Therapist to sign off on.
Your GP will also validate that your child has:
- A chronic condition; and
- Complex care needs being managed by a medical practitioner (including a general practitioner, but not a specialist or consultant physician) under a shared care plan or under both a GP Management Plan and Team Care Arrangements
